Please note, Form 1095-C will now be provided to employees upon request only. You may request your form using one of the following methods:

• Email your request to katie@asphaltpatchsystems.com
• Submit your request in writing to Asphalt Patch Systems at 8812 Canyon Road East, Puyallup, WA 98371;
• Request a copy via phone at (253) 535-2590.
